Group Home Living provides customized and assisted living in small residential homes for adults 18+ and 55+, including adults with physical and mental disabilities. Staff are on site day and night.
We take referrals and questions by email only. Email gets the fastest reply.

- Waiver type (CADI, BI, DD or EW/AC) and county
- Care needs and any safety concerns
- Preferred city and move-in timeline
- Home needs: age group, wheelchair access, pets

What daily care looks like
Our customized and assisted living program is built around each resident's routine, with help where it's needed and independence where it isn't.
Personal care
Help with bathing, dressing and grooming, given in a way that keeps each resident's own habits and privacy.
Medication assistance
Trained staff keep medication schedules on time and accurate, and track changes from prescribers.
Small-home living
A few residents per home, so staff know each person well and care plans can actually be personal.

Every resident has a care plan written with them, their family and their case manager, and we update it as needs change.

How a referral works

We review it
We check needs, waiver and location against our current openings.
We reply by email
We confirm fit and next steps, and set up a visit if there's an opening.
Move-in
We work with the case manager and family on paperwork and a move-in date.
